Treating A Dissociative Disorder

It’s often the case in the psychiatric world that people are mistakenly diagnosed with other disorders, when if fact they suffer from a dissociative disorder, either dissociative identity disorder (DID), depersonalization disorder, dissociative amnesia or dissociative fugue. Indeed, that diagnosis itself has been in and out of favor as a psychiatric diagnosis over the years, but the evidence now seems to suggest pretty strongly that it’s very real. Part of the reason why there has been some hesitation in acknowledging that these are real disorders is because the symptoms are also associated with other problems, such as depression, post traumatic stress syndrome, and eating disorders. Treatments for those problems don’t improve dissociative disorders.

People may think of an illness involving multiple personalities when they hear about a “dissociative” disorder, but the term actually covers the four different disorders, which again are dissociative amnesia, fugue (where the person wanders away to start a new life with no memory of the past), dissociative identity disorder (or DID, the one involving multiple personalities), and depersonalization. These are not anxiety disorders, though it’s true that amnesia and fugue frequently result from some kind of trauma, and DID usually develops from ongoing childhood trauma that prevents the child from integrating their personality normally while growing up.

Current treatments for a dissociative disorder pretty much always have psychotherapy as one of the elements, though hypnosis is frequently a key method for helping the amnesia patients restore the memories of a traumatic event or remember their real identity. Certain drugs, such as the medication called Pentothal (or thiopental) can also help restore memories that have been blocked out. For DID, the primary treatment is long-term psychotherapy to explore the past traumas and resolve them so the different personalities merge into one. Treating dissociative fugue, once diagnosed, often results in a fairly quick recovery, but traumatic amnesia and depersonalization can take longer to resolve. And even with the best treatments, it’s possible that DID will recur, although this often becomes less likely as the person gets older. So the prospects of success might be discouraging to some people. But given that these dissociative illnesses can manifest as clinical depression symptoms, along with eating and other anxiety disorders, it is imperative that people with one of the dissociative disorders be treated, to give themselves the best chance of recovery.

How To Treat Delusional Disorders

One disorder that the psychiatric profession finds hard to diagnose and treat is delusional disorder. Diagnosis is difficult because the delusions are considered “non-bizarre,” meaning the patient’s beliefs might theoretically be plausible, and they often function in the world. The person may not believe they have a problem, and since the most common form of the illness creates paranoia, they often refuse to find a therapist at all. Even if they do, the doctor can’t prescribe any drug products, since the delusion usually convinces the person that someone is “out to get them.” Hospitalization, too, would only reinforce that belief.

While paranoia is the most common manifestation, there are other types of delusional disorders as well, such as believing one is the secret love interest of a famous person, being convinced one has extraordinary abilities or is very important, worrying about physical problems or disfigurements that don’t exist, or believing that one’s romantic partner is unfaithful. Mental health treatment is often refused because of these convictions, which are impervious to any sort of disproof. The patient is convinced they are correct; meaning there is nothing to treat.

Only a few medical treatments have any effect for delusional disorder, and must be coupled with psychosocial treatment, which will be the primary therapy tool. Often the better treatments will approach the problem in a roundabout way, not suggesting that the patient is delusional, but instead dealing with symptoms that accompany their disorder, such as offering anxiety and depression treatment instead. Only when a trusting, respectful rapport has built up can the therapist gradually begin challenging some of the patient’s beliefs, neither condemning nor giving them credence. Drugs may be added into the mix late in the therapy, if at all, or at a point when the delusions start to become dangerous or make the patient agitated.

Therapists who are ready to use slightly different treatments, rather than adopt the more usual drugs or typical psychotherapy approaches, may gain the patient’s trust enough to begin exploring any doubts the person expresses about their own beliefs. The two of them can work in partnership, gradually discovering real-world explanations for those beliefs. If the therapist treads carefully and uses tactfully, then the two together may work through the delusional disorder and affect a cure.
